Group therapy Registration

If you are not already a current client with APP: You will be prompted to fill out our general “become a client” form at the end of the group registration form. Please fill out both to be placed in a group.

Please enter your information with the button below.

Our billing team will verify your insurance (if applicable) and then the group leader(s) will receive this information and reach out to you to begin the onboarding process. If the group you requested is full, you will be placed on the waiting list for the next one, if possible.
